Fraser Health is seeking a skilled Registered Nurse/Registered Psychiatric Nurse to join our team at Delta Hospital in Delta, BC. As a member of our multidisciplinary team, you will provide direct nursing care to patients in the Hospice Residence, assessing their physical, emotional, spiritual, and psychosocial needs, and developing and implementing individualized care plans.
Responsibilities- Assess patients' needs and develop care plans in collaboration with the healthcare team.
- Provide direct nursing care, including administering medications, performing procedures, and providing emotional support.
- Communicate effectively with patients, families, and healthcare team members to ensure seamless care.
- Participate in quality improvement initiatives to enhance patient care and safety.
- Collaborate with other healthcare professionals to provide comprehensive care.
- Document patient information accurately and timely.
- Participate in education and training programs to maintain and enhance clinical skills.
Graduation from an approved School of Nursing and/or Psychiatric Nursing. Two years' recent, related medical nursing experience in an acute care facility, including one year recent related experience in a hospice/palliative care environment, or an equivalent combination of education, training, and experience. Current practicing registration as a Registered Nurse and/or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
