Job Title: Clinical Pharmacist

We are seeking a highly skilled Clinical Pharmacist to join our team at Fraser Health Authority. As a Clinical Pharmacist, you will play a vital role in providing clinical pharmacy services to our patients, including individualized pharmacotherapy, patient medication counseling, and consultation with physicians and other healthcare providers.

Key Responsibilities:
  • Develop and implement therapeutic plans for patients, ensuring appropriate documentation in the patient health record.
  • Provide patient medication counseling, reviewing reference literature and summarizing information to meet with patients and their families.
  • Assist the healthcare team in developing drug therapy guidelines for individual disease conditions, reviewing current literature and compiling information.
  • Participate in ward rounds, program team meetings, and provision of pharmacy services at the unit level to enhance patient care.
  • Act as a department resource person, providing detailed drug information on issues related to drug therapy.
  • Contribute to the development of case studies and review articles for submission to professional journals.
  • Provide instruction and/or supervision to pharmacy students and residents, preparing educational material and demonstrating procedures.
  • Participate in quality improvement activities, reviewing current practices and performing quality assurance audits.
Requirements:
  • Current registration as a Pharmacist with the College of Pharmacists of British Columbia.
  • Completion of an accredited Hospital Pharmacy Residency Program or an equivalent combination of education, training, and experience.
  • Broad knowledge of organization policies, procedures, and standards of care.
  • Knowledge of evidence-based clinical pharmacy practice, medication use evaluation, research processes, and methodology.
  • Ability to apply clinical pharmacy knowledge and make decisions with a minimum of supervision.
  • Ability to communicate effectively both verbally and in writing.
  • Ability to establish and maintain effective working relationships with other healthcare practitioners, patients, family members, and the public.
