Job Summary

The Pharmacist will provide clinical pharmacy services in a safe, efficient, and cost-effective manner. This includes interpreting medication orders, assessing prescriptions for therapeutic suitability, and ensuring medications are accurately dispensed and/or compounded.

Key Responsibilities
  • Interpret medication orders and dispense medications using a computerized medication information system, in accordance with legal, professional, and departmental policies, procedures, and standards.
  • Assess prescriptions for therapeutic suitability by referring to standardized references, interpreting laboratory data, and reviewing patient-specific demographics.
  • Review patient profiles for any patient allergies, potential drug interactions, therapeutic duplications, or contraindications, and withhold medications until safety concerns are resolved.
  • Communicate with physicians, patients, and/or other healthcare team members for prescription clarification and problem resolution, and document the outcome of the interaction with the prescriber.
  • Provide instructions to others on proper use/administration, storage, and/or disposal, possible side effects of prescribed and self-selected medications, and communicate verbally and in writing with physicians, nurses, and other professionals.
  • Participate in obtaining medication history from select patients, verify any allergy information, and assist with medication reconciliation process.
  • Ensure that medications are efficiently and accurately dispensed and/or compounded, and labelled, and that dispensing records are completed.
  • Provide medication counselling regarding drug therapy to patients and/or their families, as required, and provide allergy assessment and discharge counselling for appropriate patients.
  • Review patient screening reports or targeted drug lists and identify potential drug-related problems based on available laboratory data.
  • Provide basic pharmacokinetic interpretations on patients receiving targeted drugs such as aminoglycoside, warfarin, and vancomycin.
  • Participate in the collection of data for medication use evaluation projects and document the required information, and assist in maintaining adequate inventories, noting shortages, requisitioning drugs and/or supplies, and monitoring expiry dates.
  • Participate in dispensary-based implementation of medication safety initiatives and quality control activities in accordance with established standards and procedures.
  • Maintain records for narcotic and controlled drugs, special access program drugs, and departmental statistics in accordance with legal, professional, and departmental standards, policies, and procedures.
  • Participate in the orientation of pharmacy students, residents, technicians, assistants, and new staff members by demonstrating dispensary-related functions, procedures, techniques, and department equipment, and providing verbal guidance and input regarding progress, as assigned.
  • Contribute to internal publications and participate in clinical drug trials by dispensing medications and completing associated documentation, and review cases of reported drug adverse reactions and complete necessary documentation.
  • Participate in staff meetings, committees, and other programs as required, and identify opportunities to improve processes and/or procedures to enable safe and efficient operations and refer to supervisor and/or others.
Qualifications
  • Current registration as a Pharmacist with the College of Pharmacists of British Columbia.
  • Demonstrates the leadership practices of the Fraser Health Leadership Framework of Clear, Caring, and Courageous, and creates the conditions for people to succeed.
  • Broad knowledge of organization policies, procedures, and standards of care.
  • Knowledge of evidence-based clinical pharmacy practice.
  • Knowledge of medication use evaluation.
  • Knowledge of research processes and methodology.
  • Knowledge of drug distribution processes in a hospital setting.
  • Ability to apply clinical pharmacy knowledge and make decisions.
  • Ability to supervise and provide work direction to staff.
  • Ability to establish and maintain effective working relationships with other health practitioners, as well as patients, family members, and the public.
  • Ability to organize and prioritize work.
  • Ability to operate related equipment, including applicable software applications.
  • Physical ability to carry out the duties of the position.
