As a key member of our interdisciplinary team, you will provide primary care to individuals who are sheltered, unsheltered, or precariously housed, presenting with complex Mental Health and Substance Use and primary care needs.
Key Responsibilities- Provide comprehensive assessments, diagnoses, and treatment plans for patients with complex health needs.
- Collaborate with clients, families, and other healthcare professionals to develop and implement individualized care plans.
- Prescribe medications and therapies as needed, while ensuring safe and effective treatment.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures.
- Stay up-to-date with the latest research and best practices in primary care and mental health.
- Current registration as a Nurse Practitioner with the British Columbia College of Nurses and Midwives (BCCNM).
- 4 years of recent, relevant clinical nursing experience.
- Successful completion of the Objective Structured Clinical Examination (OSCE).
