We are seeking a highly skilled and compassionate Registered Nurse/Registered Psychiatric Nurse to join our team at Fraser Health. As a Community Mental Health Specialist, you will play a vital role in providing treatment and care management services to individuals with mental illness and concurrent disorders.
Key Responsibilities- Client Assessment and Care Planning: Conduct thorough mental health assessments, develop and implement comprehensive treatment plans, and monitor client progress.
- Treatment and Support Services: Provide individual and group counseling, education, and support services to clients and their families.
- Interprofessional Collaboration: Work closely with the interprofessional care team to ensure seamless care coordination and communication.
- Community Outreach and Partnerships: Establish and maintain relationships with community agencies, providers, and stakeholders to ensure access to necessary services and resources.
- Documentation and Reporting: Maintain accurate and timely documentation of client assessments, care plans, and progress.
- Education and Experience: Graduation from an approved School of Nursing or Psychiatric Nursing, with a minimum of two years of experience in mental health and/or substance use treatment.
- Registration and Certification: Current practicing registration as a Registered Nurse and/or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
- Skills and Abilities: Strong assessment and treatment planning skills, excellent communication and interpersonal skills, and the ability to work effectively in a fast-paced environment.
