Job Summary

Key Responsibilities

  • Provide assessment and consultation services to patients and families
  • Develop and implement individualized treatment plans
  • Collaborate with the interprofessional team to ensure comprehensive care
  • Participate in program development and evaluation
  • Attend regular team meetings and participate in inter-professional patient and family conferences

Requirements

  • Registration and license to practice with the College of Physiotherapists of Ontario
  • Recent experience in acute care hospital-based medicine and geriatric care
  • Excellent interpersonal and communication skills
  • Ability to work effectively and efficiently in a fast-paced environment
  • Strong interpersonal, organizational, and problem-solving skills
