Vancouver Coastal Health is seeking a skilled Home Support Nurse (RN) to join our team in Vancouver, BC.
This is a challenging and rewarding role that requires a strong commitment to providing high-quality patient care in a home setting.
Key Responsibilities- Care for clients with acute, chronic, and palliative care needs, providing short or long-term home support services.
- Work in partnership with clients and families to establish goals, assess, plan, coordinate, organize, implement, evaluate, and transition nursing and other services.
- Assess service eligibility, facilitate the development, implementation, and adaptation of a care plan, deliver direct care, and provide care management to assist clients to manage their own care and navigate through the various services available within the community settings.
- Work in consultation with other system partners and incorporates current evidence into practice.
- Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
- Two years' recent clinical experience that is relevant to the specific clinical area of employment or equivalency of previous work experience will be considered.
- Valid BC Driver's License required as local area travel may require the use of a personal vehicle.
