About the Role

CarePartners is seeking a skilled Physiotherapist to join our dynamic Rehabilitation Team. As a Physiotherapist, you will play a critical role in providing high-quality patient care in the community and in patients' homes.

Key Responsibilities

  • Conduct professional assessments of patients in the community and in their homes
  • Develop and maintain effective Care Plans with respect to Community Health Services
  • Work with patients to set and achieve time-specific, measurable goals and outcomes in collaboration with the care team
  • Provide active guidance for workplace Health and Safety
  • Perform other duties as required

Requirements

  • Master's Degree (or equivalent) degree in Physiotherapy from a recognized university
  • Registration with the College of Physiotherapists of Ontario
  • Proven computer skills, including email, word processing, and work scheduling systems
  • Strong assessment skills and the ability to participate in effective treatment programs
  • Excellent English verbal and written communication skills
  • French language skills are an asset
  • Clear Background and Vulnerable Sector Check
