Home Health Care Clinical Supervisor

We are seeking a highly skilled and experienced Clinical Community Supervisor to join our team at ParaMed Home Health Care. As a Clinical Community Supervisor, you will play a critical role in the supervision of Personal Support Workers (PSWs) and Health Care Aides (HCAs) who provide care and support to clients in their homes.
Key Responsibilities- Supervise and support PSWs and HCAs to ensure they are providing high-quality care and services to clients in accordance with our company's policies and procedures.
- Respond to and resolve concerns from clients, families, and other team members in a timely and professional manner.
- Provide ongoing leadership and guidance to PSWs and HCAs through regular meetings, coaching, and mentoring.
- Initiate and investigate patient safety incidents, and develop and implement action plans to prevent future incidents.
- Collaborate with the nursing team to ensure accurate and timely clinical documentation and submission of reports to funders.
- Engage clients in problem-solving and health teaching, and build rapport and trust to identify their health care needs, strengths, capacities, and goals.
- Engage staff in problem-solving, education, coaching, and mentoring to resolve problems and improve practice.
- Maintain current knowledge, skills, and attitudes to provide safe, efficient, and effective service in all areas of practice.
- Current RPN registration with the College of Nurses of Ontario and entitled to practice without restrictions.
- Minimum of 5 years' experience as a practicing Registered Practical Nurse.
- Comprehensive patient health assessment skills.
- Understanding of the principles of community nursing practice.
- Proven experience leading and managing a team.
- Knowledge of clinical documentation standards and practice guidelines.
- Excellent organizational, interpersonal, and teaching skills.
- Commitment to professional development in clinical practice relevant to home health care.
- Effective critical thinking and problem-solving skills.
- Valid driver's license and access to reliable transportation.
- Able to provide a current, clear Vulnerable Sector Screening (VSS).
- C.N.A. Certification as a Community Health Nurse (CCHN(C)) is considered an asset.
