About the Role

CarePartners is seeking a skilled and compassionate Registered Nurse (RN) or Registered Practical Nurse (RPN) to join our team as a Community Health Nurse. As a Community Health Nurse, you will have the opportunity to work in a variety of settings, including in-home, clinics, retirement homes, and schools, providing exceptional nursing services to patients of all ages.

Key Responsibilities
  • Develop and maintain effective care plans with respect to community health services, independently or in cooperation with the Home and Community Care Support Services (HCCSS).
  • Work with patients to provide exceptional nursing services, both in-person and virtually.
  • Provide services in a variety of settings, including in-home, clinic, retirement home, or school.
  • Communicate and liaise with patients, family members, and others within the interdisciplinary care team.
  • Perform other clinical duties as required.
Requirements
  • Registered Nurse (RN) or Registered Practical Nurse (RPN) degree or diploma.
  • Current College of Nurses of Ontario (CNO) registration in good standing.
  • A strong desire and commitment to making a difference in the lives of our patients.
  • Exceptional interpersonal skills.
  • A valid driver's license and vehicle, as regular travel is required.
  • Excellent English verbal and written communication skills.
  • CPR Certification.
  • Current immunization record, including a 2-step TB test.
  • To protect our patients, we require a current vulnerable sector check and a clear background check.
