About the Role

We are seeking a skilled Licensed Practical Nurse/Registered Practical Nurse to join our team at Bayshore HealthCare. As a key member of our home care team, you will provide high-quality care to clients in the comfort of their own homes.

Key Responsibilities
  • Provide compassionate and individualized care to clients, prioritizing their emotional, physical comfort, and safety.
  • Develop and implement customized care plans in collaboration with clients, families, and healthcare teams.
  • Conduct thorough assessments, identify client needs, and provide education and support to clients and their families.
  • Collaborate with healthcare professionals to ensure seamless care transitions and coordination.
  • Participate in ongoing training and professional development to enhance skills and knowledge.
Requirements
  • Graduation from an accredited Licensed/Registered Practical Nursing Program.
  • Current registration with the provincial licensing body.
  • Valid CPR certification.
  • At least two years of recent experience in home care or a related field.
  • Reliable mode of transportation or a valid driver's license and reliable vehicle.
