Physiotherapist - Community Health Specialist

Job Summary
Community Therapists is seeking a skilled Physiotherapist to join our team in a community-based position. As a Physiotherapist - Community Health Specialist, you will be responsible for providing high-quality patient care and leading a team of rehab assistants in a community setting.
Key Responsibilities
- Conduct in-home and community-based assessments to identify patient needs and develop personalized care plans.
- Supervise and mentor rehab assistants to ensure they have the necessary skills and knowledge to provide excellent patient care.
- Develop and oversee gym and pool-based activation programs to promote patient mobility and independence.
- Liaise with healthcare teams, including occupational therapists and physicians, to ensure seamless patient care.
Requirements
- Current membership with the College of Physiotherapists of British Columbia (CPTBC) or eligibility for registration.
- Valid driver's license and access to a reliable vehicle.
- Experience with standardized assessments and a strong understanding of community-based care.
- Excellent written and verbal communication skills to effectively collaborate with patients, families, and healthcare teams.
- Experience working with insurance payers is an asset.
