Key Responsibilities- Provide direct patient care to clients receiving scheduled or urgent primary care services
- Assess clients and collect information from various sources to inform nursing diagnoses and interventions
- Develop and implement individualized care plans in collaboration with the interprofessional team and clients
- Provide health education and counseling to clients and their families
- Collaborate with other healthcare professionals to ensure seamless care transitions and coordination
- Participate in quality improvement initiatives and contribute to the development of policies and procedures
- Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives
- One year recent related clinical experience in acute care, urgent/emergency care, or a related clinical area
- Valid BC Driver's license and access to a personal vehicle for business-related purposes
