Responsibilities:
* Provide comprehensive nursing care to patients in their homes, including wound care, IV medications, and re-sites, catheter care, compression/VAC and ostomy care
* Collaborate with patients, families, and healthcare professionals to develop and implement individualized care plans
* Conduct thorough assessments of patients' physical and emotional needs, identifying areas for improvement and developing strategies for addressing them
* Educate patients and families on self-care techniques and provide support and guidance as needed
* Maintain accurate and detailed records of patient interactions and care provided
Requirements:
* Bachelor of Science in Nursing degree (BScN) or equivalent
* Current registration with the College of Nurses of Ontario
* Valid Standard First Aid and Level C CPR certification
* Ability to work independently and as part of a team
* Excellent communication and interpersonal skills
* Ability to use phone applications
