Job Summary
Mackenzie Health is seeking a highly skilled and experienced Clinical Social Worker to join our Mental Health Day Hospital Program team. As a key member of our interprofessional team, you will provide high-quality patient-centered care to individuals experiencing mental illness.
Key Responsibilities
- Develop and lead therapeutic groups to enhance coping skills and improve functioning.
- Provide individual psychosocial assessments, counseling, and treatment planning.
- Coordinate discharge planning and collaborate with the interdisciplinary team to establish priorities.
- Develop and maintain communication resource linkages and networks.
- Engage in reflective practice and seek guidance to support continued competence.
Requirements
- Master's degree in social work from an accredited university.
- Member of the Ontario College of Social Workers and Social Service Workers.
- Minimum of 3 years working in an acute mental health environment.
Preferred Qualifications
- Previous experience working in a hospital environment.
- Minimum of 3 years current related clinical experience.
