Key Responsibilities- Provide professional assessment of patients in the community and in their homes
- Develop and maintain effective Care Plans with respect to Community Health Services
- Work with patients to establish time-specific, measurable goals and outcomes in collaboration with the care team
- Provide active guidance for workplace Health and Safety
- Perform other duties as required
- Master's Degree (or equivalent) in Occupational Therapy from a recognized university
- Registration with the College of Occupational Therapists of Ontario
- Proven computer skills, including email, MS Word and Outlook, work scheduling systems
- Strong assessment skills and the ability to participate in effective treatment programs
- Excellent English verbal and written communication skills
- French language skills are an asset
- Clear Background and Vulnerable Sector Check
