Qualifications: Education, Training, and Experience

A Bachelor's Degree in Social Work from an accredited institution is required. Candidates must hold current full registration with the British Columbia College of Social Workers. A minimum of two years of recent relevant experience in a healthcare environment, particularly with medically complex clients facing concurrent disorders, is essential. Alternatively, a combination of education, training, and experience demonstrating comprehensive knowledge of chronic health issues, geriatrics, community health principles, human development, behavior, and family/group dynamics will be considered. Familiarity with applicable legislation and a solid understanding of available programs and services, their interrelationships, and their roles in community care delivery is also necessary.

A valid BC Driver's License is required.

Skills and Abilities
  • Proven counseling abilities, including expertise in crisis, loss, and grief counseling.
  • Ability to assess and adapt to evolving situations with patients in a flexible manner.
  • Capacity to foster effective relationships with primary care providers, interdisciplinary teams, and community partners to promote a collaborative approach to patient care.
  • Basic computer proficiency is required.
  • Knowledge of primary health care principles, community development strategies, and the strategic objectives of the Vancouver Island Health Authority concerning primary health care and chronic disease management.
  • Familiarity with facilitation, mediation, and conflict resolution techniques.
  • Strong analytical and problem-solving capabilities.
  • Experience in case management, clinical support, and consultation.
  • Ability to prioritize tasks, work independently, and adopt a cooperative, proactive approach to patient care.
  • Competence in collaborating effectively within a multidisciplinary team, demonstrating initiative and independence.
  • Effective communication skills with team members, primary care providers, patients, their families, and caregivers.
  • Ability to serve as a clinical resource and educator, applying adult learning principles.
  • Strong organizational skills to manage workload efficiently.
