Patient Care Support Specialist

Position Overview
Halton Healthcare is seeking a dedicated individual to join our team as a Patient Care Support Specialist. In this role, you will be responsible for providing patient observation, supporting clinical staff, and ensuring the well-being of our patients within a mental health setting.
Responsibilities
- Provide patient observation/supervision based on nurse/physician assessment and promptly alert assigned nurses about any changes in a patient's presentation.
- Meticulously document observations as required by physician orders.
- Carry out routine patient checks to monitor their condition and well-being.
- Accompany patients for transfers and appointments, ensuring their safe and comfortable movement throughout the facility.
- Provide support to both patients and clinical staff during ward activities, contributing to a positive and efficient environment.
- Engage patients in recreational activities to promote their mental stimulation and social interaction.
- Assist with admissions, discharges, and transfers, ensuring a smooth transition for patients.
- Provide assistance and supervision to patients during dining room service, promoting independence and socialization.
- Answer phone calls and inquiries from patients and their families, providing clear and compassionate information.
- Demonstrate proficiency in your chosen profession and adhere to all relevant ethical and professional standards.
Qualifications
- Proven ability to perform patient restraints safely and effectively, adhering to established protocols.
- Post-secondary diploma in a related field, such as healthcare or social work.
- Education or experience specifically related to mental health or healthcare settings is highly desirable.
- Excellent observation skills with the ability to identify subtle changes in patient behavior and condition.
- A strong understanding of mental health issues and the challenges faced by individuals experiencing these conditions.
- Experience working with patients displaying escalated behaviors, demonstrating de-escalation techniques and conflict resolution skills.
- Exceptional communication skills, both written and verbal, enabling clear and empathetic interactions with patients, families, and colleagues.
- Current First Aid and CPR certification to ensure the safety of patients in emergency situations.
- Completion of a Prevention and Management of Aggressive Behaviour Course to effectively handle challenging patient behaviors.
- Proficiency in computer skills and Microsoft Office Suite for documentation and communication purposes.
