**Job Summary**

We are seeking a skilled Family Practitioner to join our team at Island Health, providing outreach services in Sooke, B.C. This part-time (0.2 FTE) role will involve delivering primary care services to adults and children impacted by poverty, colonization, stigma, and homelessness.

**Key Responsibilities**

  • Provide accessible primary episodic care services that address immediate medical needs and work towards decreasing barriers and social determinants of health in accessing care.
  • Collaborate with a primary care RN, an MHSU RN, and a MOA to provide OAT and primary care services to a broad range of clients.
  • Develop and implement strategies to improve health outcomes and reduce health inequities in the community.

**Requirements**

  • Family Medicine certification or equivalent experience.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
