Job Summary
We are seeking a compassionate and skilled Registered Nurse to join our Home Care Program. In this role, you will provide essential nursing care to patients in their homes, ensuring their well-being and promoting their independence.
Responsibilities
- Deliver personalized nursing care to patients within the home setting, adhering to established protocols and best practices.
- Develop and implement comprehensive care plans tailored to individual patient needs, collaborating with healthcare professionals and families.
- Monitor patient health status, assess their progress, and make necessary adjustments to care plans.
- Educate patients and their families on various health conditions, medications, and self-care techniques.
- Maintain accurate and detailed patient records, documenting all interactions and interventions.
- Participate in program development and evaluation activities, contributing to the continuous improvement of home care services.
Qualifications
- Registered Nurse with a valid license to practice in the province/territory.
- Experience working in isolated and intercultural environments is highly desirable.
- Knowledge of home care principles and practices.
- Excellent communication, interpersonal, and problem-solving skills.
- Ability to work independently and as part of a multidisciplinary team.
- Bilingual proficiency in French and English; Inuktitut language skills are an asset.
