Job Title: Continuing Care Assistant

Job Summary:

We are looking for a compassionate and detail-oriented Continuing Care Assistant to provide high-quality care to our clients in their homes and communities. As a Continuing Care Assistant, you will work collaboratively with our healthcare team to deliver personalized care and support to individuals with complex needs.

Responsibilities:
  • Provide personal care and support to clients, including bathing, dressing, and grooming
  • Assist with daily living activities, such as meal preparation and medication management
  • Support clients with mobility and transfer needs, including lifting and transferring
  • Deliver messages, reports, and requisitions between departments
  • Maintain a safe and clean environment for clients
  • Collaborate with healthcare professionals to develop and implement care plans
  • Provide emotional support and companionship to clients
Requirements:
  • Continuing Care Assistant Certification
  • First Aid Certificate
  • CPR Certificate
  • Criminal record check
  • Child Abuse Registry check
  • Immunization records
  • Vulnerable sector check
