Job Summary

We are seeking an experienced Advanced Practice Clinician to join our team at Waypoint Centre for Mental Health Care. The successful candidate will be responsible for providing clinical leadership and expertise in geriatric mental health, working closely with our regional partners to improve patient outcomes and build capacity.

Key Responsibilities
  • Implement the recognized competencies of an advanced practice clinician role: clinical practice, education/mentoring and leadership.
  • Provide excellence in clinical practice in the care of older adults and their caregivers, including leading the development and implementation of clinical best practices, practice guidelines, standardized protocols, etc.
  • Support the development, implementation and evaluation of clinical services and programs to improve the quality of care provided to older adults and their caregivers.
  • Provide consultative support to local teams/partners to advance clinical practice and build system capacity.
  • Educate and mentor members of interprofessional teams and other clinicians across the region to build regional and local capacity in the care of older adults and caregivers.
  • Promote a learning environment for older adults, caregivers, health service providers, health care students, leaders and policy makers.
  • Use research and leading practices to inform clinical care, education and planning. As appropriate, lead or participate in quality improvement initiatives to influence clinical practice, planning and/or policy.
  • Participate in data and information analysis to identify and address gaps in care, issues in practice, risk issues and/or concerns impacting the safety of older adults or their caregivers.
  • Represent the program locally, regionally and provincially.
  • Build strong collaborative relationships with key internal and external partners and stakeholders.
Requirements and Qualifications
  • Regulated Health Professional with a Masters Degree and registration in good standing with no restrictions.
  • P.I.E.C.E.S., Gentle Persuasion Approach (GPA), Positive Approach to Care (PAC), Montessori training preferred.
  • Current certification in Gerontology or Geriatrics (i.e. academic institution, GNC(C) from the Canadian Nurses Association).
  • Membership in professional associations and/or interest groups an asset.
  • Valid Ontario driver's license and operational vehicle required.
  • Minimum three years recent experience working with older adults and caregivers essential.
  • Minimum two years recent experience working within a Specialized Geriatric Services team in geriatric mental health essential; experience in a regional role preferred.
  • Experience in advanced clinical practice with the ability to adapt to change, generate new knowledge and apply it in different ways to formulate and problem solve within a context of complexity and uncertainty is essential.
  • Demonstrated skill in group facilitation, knowledge-to-practice and adult learning theory in order to provide education, coaching and mentoring to colleagues providers and caregivers.
  • Proven success required in:
    • Clinical assessment, intervention and evaluation.
    • Interpersonal engagement and team-building.
    • Decision-making, critical thinking and conflict resolution.
    • Risk management and optimizing patient safety.
    • Influencing change at a service, organization and system level.
    • Project management and program planning/implementation/evaluation.
    • Managing competing demands and priorities within defined timelines.
    • Data and research analysis, including survey methodology.
    • Innovation, creative approaches to clinical and thinking outside the box.
    • Verbal and written communication, including effective use of computer programs like Microsoft Office suite.
    • Working autonomously and being self-directed.
    • Results oriented work ethic to work independently and undertake tasks needed to accomplish work objectives and deliver quality, consistent and timely results
    • Demonstrated teamwork, interpersonal and organizational skills
    • Proven written and verbal communication skills; and a proven ability to adapt readily to change
    • Models and promotes core ethical practice, Waypoint Values, and reflects an optimistic and positive attitude
    • Preference for proficiency in French/English language skills
