Our multi-site department provides a comprehensive range of services across inpatient, outpatient, and retail pharmacy sectors. We operate pharmacy facilities at various locations, ensuring a wide array of specialty patient services, including neonatal, pediatric, adult, geriatric, and palliative care, while also participating in clinical trials and research initiatives.

Position Overview

We are seeking a dedicated pharmacist to contribute to our mission of ensuring safe and effective medication use across clinical units. The pharmacy department is integral to the safe distribution and management of medications, which is crucial for achieving positive patient treatment outcomes in accordance with legal, regulatory, and accreditation standards. The pharmacist will play a vital role within the multidisciplinary healthcare team, focusing on the following responsibilities:

  • Providing clinical pharmacy services aimed at enhancing patient quality of life.
  • Reviewing physician orders and maintaining patient medication profiles, which may include IV admixture, TPN, pediatric and chemotherapy preparation, and drug therapy monitoring.
  • Entering medication orders with a future transition to computerized physician order entry systems.
  • Engaging in educational activities for students, families, and healthcare staff.
  • Collaborating with the healthcare team to optimize medication use and prevent medication-related issues.

Work Schedule

Shifts may include days, evenings, nights, weekends, and holidays.

Qualifications

1. Bachelor of Science in Pharmacy is required; PharmD is an asset.
2. Current registration with the Ontario College of Pharmacists is mandatory.
3. Previous hospital experience in unit dose and/or IV admixtures is preferred.
4. Completion of a hospital residency program or equivalent is an asset.
5. Ability to work independently and collaboratively within a team environment.
6. Strong organizational skills with the ability to manage a varied workload.
7. Effective communication skills, both written and verbal, to facilitate interactions with patients and healthcare providers.
8. Proficiency in identifying, preventing, and resolving drug-related issues.
9. Active membership in the Canadian Society of Hospital Pharmacists (CSHP) is advantageous.
10. Understanding of change management principles and the ability to act proactively.
11. Leadership and problem-solving skills are essential.
