Your Opportunity:

As a Social Worker II with Alberta Health Services, you will play a vital role in providing a range of social work services to patients and their families in the community of Whitecourt and surrounding areas. Working collaboratively with an interdisciplinary healthcare team, you will deliver services to patients and families across the healthcare continuum, including acute care, continuing care, supportive living, and community health sectors.

Key Responsibilities:
  • Provide bio-psychosocial assessment, resource navigation, crisis intervention, education, supportive and adjustment counselling, advocacy, and consultation services.
  • Make appropriate referrals to internal and external services, working collaboratively with community agencies and programs to benefit patients and families.
  • Prioritize workload, analyze complex problems, and identify viable care plans and solutions to address bio-psychosocial needs.
  • Contribute to non-patient care activities, including program planning, evaluation, quality improvement research, and staff development.
Requirements:
  • Completion of a bachelor's degree in Social Work.
  • Active or eligible for registration with the Alberta College of Social Workers (ACSW).
  • Prior experience working within a healthcare environment is an asset.
  • Knowledge of decision-making legislation and capacity assessment process is preferred.
  • In-depth knowledge of Income Support programs at a community/provincial/federal level (AISH, Alberta Works, CPP, OAS, ASB) is preferred.
