Registered Dietitian - Home and Community Care

We are seeking a compassionate and dedicated Registered Dietitian to join our growing team in the St. Thomas and Elgin County area. This role offers the opportunity to provide personalized nutrition care to individuals within their own homes, fostering independence and well-being.

Responsibilities:
  • Conduct comprehensive nutritional assessments to identify client needs and develop individualized treatment plans.
  • Provide evidence-based dietary counseling and education to clients, addressing a range of health conditions and concerns.
  • Demonstrate expertise in evaluating and managing swallowing disorders, ensuring safe and effective nutrition delivery.
  • Collaborate effectively with interdisciplinary teams, including physicians, nurses, and social workers, to provide holistic care.
  • Maintain accurate and timely documentation of client interactions and progress.
Qualifications:
  • Bachelor's degree in Food and Nutritional Sciences or equivalent.
  • Current active registration in good standing with the College of Dietitians of Ontario.
  • Experience working with elderly populations and providing home-based care.
  • Proficiency in tube feeding and Total Parenteral Nutrition (TPN) management.
  • Strong clinical assessment and problem-solving skills.
  • Valid Ontario Driver's License and reliable transportation.
