Job Summary

We are seeking a skilled Nurse Practitioner to join our team at Island Health as a Community Health Specialist. This is an exciting opportunity to provide primary care to individuals and families in the Western Communities.

About the Role

As a Nurse Practitioner, you will be an integral member of our multidisciplinary Primary Care Team, providing longitudinal primary care to individuals of all ages and their families. You will work with a focus on those who experience barriers to health/health care due to the impact of inequity and the Social Determinants of Health (SDOH).

Key Responsibilities

  • Provide primary care to individuals and families in the Western Communities
  • Work collaboratively with a multidisciplinary team to provide coordinated care
  • Develop and implement care plans that address the unique needs of each patient
  • Conduct health assessments, diagnose and treat illnesses, and provide health education
  • Collaborate with community resources to address social determinants of health

Requirements

To be successful in this role, you will have:

Current registration with the British Columbia College of Nurses and MidwivesExperience working in a primary care settingStrong communication and interpersonal skillsAbility to work independently and as part of a team
