We are seeking a skilled Speech-Language Pathologist to join our team at Home and Community Care Support Services. As a valued member of our team, you will play a crucial role in providing high-quality care and support to our clients in their homes and communities.

Key Responsibilities:
  • Assessment and Treatment Planning: Conduct comprehensive assessments of clients' speech and language needs, and develop personalized treatment plans to address their goals and objectives.
  • Therapy Services: Provide one-on-one therapy sessions to clients in their homes, schools, and nursing homes, using evidence-based practices and techniques.
  • Collaboration and Communication: Work closely with clients, caregivers, and other healthcare professionals to ensure seamless care coordination and effective communication.
  • Documentation and Reporting: Maintain accurate and detailed records of client progress, and provide regular reports to stakeholders as required.
  • Professional Development: Stay up-to-date with the latest research, best practices, and industry developments to ensure ongoing professional growth and expertise.
Requirements:
  • Education and Certification: Master's degree in Speech-Language Pathology and current certification with the College of Audiologists and Speech-Language Pathologists of Ontario.
  • Experience: At least two years of experience in a similar community-based setting, with a strong track record of providing high-quality care and support to clients.
  • Skills and Abilities: Excellent communication and interpersonal skills, with the ability to work effectively with diverse client populations and healthcare teams.
  • Language Proficiency: Fluency in English and French is an asset.
  • Other Requirements: Valid driver's license and access to a vehicle, as well as a valid N95 Mask Fit Test result or documentation of a medical exception within the past two years.
