Job Summary

We are seeking a highly skilled Physician - Geriatric Specialist to join our team at TEEMA Group. As a key member of our healthcare team, you will provide direct primary medical care to patients and supervise medical care personnel.

Key Responsibilities
  • Conduct comprehensive history and physical assessments for new referrals, ensuring accurate and timely diagnosis and treatment.
  • Perform interval history and physical exams for established participants, conducted quarterly or as needed, to monitor their health and well-being.
  • Evaluate and treat participants with episodic illnesses, providing prompt and effective care.
  • Make referrals to Medical Specialists as necessary, ensuring seamless coordination of care.
  • Admit participants to the hospital when appropriate, providing primary care responsibilities during hospitalization and coordinating discharge planning with the Interdisciplinary Team.
  • Oversee the care of participants residing in nursing homes, including regular visits and communication with nursing home staff.
  • Engage in family conferences for care planning and implementation, ensuring that patients and their families are informed and involved in their care.
  • Coordinate 24-hour care delivery, ensuring that patients receive continuous and comprehensive care.
  • Document participant changes accurately in medical records and communicate these changes to the Interdisciplinary Team promptly.
  • Participate in staff meetings and contribute to participant care planning, ensuring that all team members are informed and aligned.
  • Stay updated on quality/clinical standards and best practices for the geriatric population, ensuring that care is evidence-based and patient-centered.
Requirements
  • Valid MD/DO license in good standing to practice medicine in the State of practicing.
  • Board Certification in Geriatric Medicine.
  • Graduate of an accredited School of Medicine.
  • Minimum of two years of clinical experience required.
  • Experience working with a frail or elderly population preferred, or willingness to undergo appropriate training upon hiring.
  • Knowledge of geriatric medicine and the special needs of complex geriatric patients.
  • Fellowship or additional training in Geriatrics preferred.
  • Understanding of the culture and needs of socially and ethnically diverse populations.
