Clinical Pharmacist

We are seeking a highly skilled Clinical Pharmacist to join our team at Island Health. To be successful in this role, you will need to be licensed by the College of Pharmacists of British Columbia or eligible for licensing.
Additionally, you will require completion of an accredited pharmacy residency and two years of recent experience as a pharmacist in a hospital setting, providing direct patient care or an equivalent combination of education, training, and experience. A valid BC driver's license is also necessary.
Key Responsibilities- Identify and resolve drug therapy problems to ensure optimal patient outcomes.
- Apply clinical knowledge to make sound decisions with minimal supervision, prioritizing patient care and safety.
- Work collaboratively as part of a multidisciplinary healthcare team to deliver high-quality patient care.
- Establish and maintain effective working relationships with physicians, other healthcare professionals, patients, and family members.
- Communicate effectively both verbally and in writing, ensuring clear and concise information exchange.
- Be self-directed, organizing and prioritizing workload to complete tasks accurately, precisely, and dependably.
- Teach and educate patients, families, and healthcare professionals on medication management and related topics.
- Operate related equipment and utilize standard office software, such as Word and Excel.
- Perform physical duties associated with the role, ensuring safe and efficient patient care.
