Job Summary

We are seeking a highly skilled and compassionate LPN to join our team. As an LPN, you will be responsible for providing high-quality nursing care to our residents, including assessing their needs, developing and implementing care plans, and collaborating with other healthcare professionals to ensure seamless care delivery.

Key Responsibilities
  • Assess and Evaluate Residents

Conduct thorough assessments of residents' physical and emotional needs, identifying areas for improvement and developing targeted care plans to address these needs.

Develop and Implement Care Plans

Work with residents, families, and other healthcare professionals to develop and implement individualized care plans that meet the unique needs of each resident.

Provide Direct Care

Deliver high-quality nursing care to residents, including administering medications, performing wound care, and providing emotional support and comfort.

Collaborate with Healthcare Team

Work closely with other healthcare professionals, including physicians, therapists, and social workers, to ensure seamless care delivery and address any concerns or issues that may arise.

Document Resident Information

Accurately and thoroughly document resident information, including medical history, treatment plans, and progress notes.

Requirements
  • Eligibility for Licensure

Must be eligible for licensure as a practical nurse in the province/territory of work.

Geriatric Experience

Experience working with geriatric patients is an asset.

Certifications and Licenses

Must hold current certifications and licenses as required by the province/territory of work.
