Residential Rehabilitation Counselor

Reporting to the Small Options Homes Director, the Residential Rehabilitation Counselor plays a vital role in ensuring the well-being and progress of our clients. This position requires a dedicated individual with a passion for helping others and a commitment to providing high-quality care.

Responsibilities
  • Client Well-being: Ensure the physical and mental health of all clients is maintained, observing and documenting any changes in their condition. Accompany clients to appointments as needed and promote healthy habits such as personal hygiene and nutrition.
  • Program Development and Implementation: Identify client needs and collaborate with the program team to develop, implement, and evaluate individualized programs that foster independence and growth. Teach essential life skills, including independent living, household management, socialization, and more.
  • Client Support and Engagement: Provide ongoing support and assistance to clients in their daily activities and responsibilities. Encourage positive interactions and interpersonal relationships within the home and community. Facilitate access to recreational opportunities and accompany clients on outings as required.
  • Communication and Collaboration: Maintain clear communication with the House Coordinator, staff team, clients, families, and external resources. Prepare comprehensive reports, including assessments, program plans, progress updates, and incident reports.
  • Safety and Maintenance: Ensure the home environment is clean, safe, and well-maintained. Implement safety protocols and respond effectively to crisis situations. Adhere to all relevant policies and procedures.
Qualifications

The ideal candidate will possess excellent verbal and written communication skills, a patient and empathetic demeanor, and strong organizational abilities. Experience working with individuals in need of support is highly desirable. A commitment to teamwork, independent decision-making, and adaptability are essential qualities for success in this role.
