Nurse Practitioner

We are seeking a compassionate and skilled Registered Nurse to join our Palliative Care team at SE Health. As a Registered Nurse, you will play a vital role in providing high-quality care to individuals and their families, focusing on improving their quality of life.

• Provide palliative care services to individuals and their families, addressing their physical, emotional, and spiritual needs.
• Work collaboratively with a multidisciplinary team, including physicians, personal support workers, and other healthcare professionals.
• Develop and implement individualized care plans, taking into account the patient's goals, values, and preferences.
• Conduct thorough assessments, identify care needs, and provide education to patients and families.
• Participate in quality improvement initiatives, contributing to the development of best practices and policies.
