Position: Access and Discharge Coordinator
Company: Vancouver Coastal Health (VCH)
Overview: Vancouver Coastal Health is on the lookout for a dedicated Registered Nurse (RN) or Registered Psychiatric Nurse (RPN) to become part of the Provincial Neuropsychiatry Program at UBC Hospital in Vancouver, BC, taking on the role of Access and Discharge Coordinator.
Key Responsibilities:
- Establish and manage a centralized referral and access utilization framework.
- Oversee and sustain a bed occupancy and waitlist management system.
- Act as a bridge between service providers and healthcare teams.
Qualifications:
Education & Experience:
- Current registration as a Registered Nurse or Registered Psychiatric Nurse.
- A minimum of two years of recent, relevant nursing experience.
- Possession of a valid BC Driver's License.
Knowledge & Skills:
- Thorough understanding of nursing science theory and its practical applications.
- Effective communication skills with colleagues and other healthcare professionals.
- Physical capability to fulfill the responsibilities of the role.
