Role Overview: The Licensed Practical Nurse (LPN) plays a crucial role in delivering exceptional care and support to residents, their families, and volunteers, adhering to established policies and procedures.

The LPN provides services that are attuned to the holistic needs of residents, promoting choice while upholding dignity and fostering independence.


Resident-Centered Care:

Ensures service excellence by being responsive to residents' needs as outlined in their care plans, policies, and procedures.

Conducts thorough nursing assessments to establish and prioritize care, collaborating effectively with Registered Nurses and other Interdisciplinary Team members.

Accurately documents resident care assessments, interventions, and outcomes in accordance with Bethany policies and regulatory requirements.

Identifies and addresses unsafe conditions in the work environment, reporting any issues immediately.

Embodies the vision and values of Bethany, actively participating in Interdisciplinary Team meetings and advocating for resident choice.

Engages in educational opportunities, including health and safety training.


Team Collaboration:

Actively participates in fostering a responsible atmosphere of resident-centered care alongside the Interdisciplinary Team.

Encourages constructive relationships within the team, recognizing the unique contributions of all members, as well as with residents, families, and visitors.


Leadership:

Facilitates a high-performance culture by leading, developing, and coaching team members. Provides direction and supervision to Health Care Aides and assumes the 'In Charge' role when necessary.


Qualifications:
  • Graduate of a recognized Licensed Practical Nursing program
  • Previous experience as a Licensed Practical Nurse is preferred
  • Demonstrates proficiency in English, including speaking, reading, and writing
  • Dementia Care Certificate and experience preferred
  • Basic computer skills and familiarity with electronic documentation are preferred

Classification: Licensed Practical Nurse (LPN)
Unit: ALC Level 4
Position Type: Part-Time
Shifts per Cycle: 10 shifts in a 5-week cycle
