Job Summary

We are seeking a Registered Nurse/Registered Practical Nurse to join our team in the Perth Region. As a member of our team, you will have the opportunity to work with a diverse range of clients, providing personalized care and support to help them achieve their goals and maintain their independence.

Responsibilities

• Provide high-quality nursing care to clients in their homes, including wound care, medication management, and vital sign monitoring

• Develop and implement individualized care plans in collaboration with clients, families, and healthcare teams

• Conduct thorough assessments of clients' physical and emotional needs, identifying areas for improvement and developing strategies to address them

• Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure comprehensive care

• Maintain accurate and up-to-date records of client care, including progress notes, medication lists, and treatment plans

Requirements

• Current registration with the provincial licensing body

• Valid driver's license and reliable vehicle

• Strong communication and interpersonal skills, with the ability to work effectively with clients, families, and healthcare teams

• Ability to work independently and as part of a team, with a high level of flexibility and adaptability

• Strong problem-solving and critical thinking skills, with the ability to analyze complex situations and develop effective solutions
