Job Summary

We are seeking a highly motivated and dedicated Registered Nurse to join our team in Primary Care at Northern Health. As a Primary Care Nurse, you will play a vital role in delivering compassionate patient care and working collaboratively with our interprofessional team to provide high-quality services to our patients.

Key Responsibilities
  • Provide direct clinical care and services to individuals, groups, and communities in a variety of settings, including physician practices, homes, schools, and communities.
  • Develop and implement care plans that are tailored to the unique needs of each patient, in collaboration with the interprofessional team.
  • Conduct initial and ongoing client assessments, and provide nursing care through appropriate and prescribed technical, therapeutic, and safety-type interventions.
  • Communicate effectively with patients, families, the public, medical staff, and the members of the interdisciplinary team using verbal, written, and computer communication means.
  • Participate in team huddles and other activities to promote communication, cooperation, and collaboration across disciplines to facilitate continuity of care.
Requirements
  • Three years recent, related clinical nursing experience in primary care nursing and/or chronic disease management settings, including experience developing and providing formal education to patients, families, the general public, or an equivalent combination of training and experience.
  • Driver's License.
