REGISTERED DIETITIAN PT

We are seeking a part-time Registered Dietitian to join our team at Extendicare Tri-Town, a 60-bed Long Term Care Home in Haileybury, Ontario. The successful candidate will provide nutritional care services to residents, working within legislative and budgetary parameters.

Responsibilities:

• Collaborate with the Clinical Team to develop and implement nutritional care plans for residents, ensuring their nutritional needs are met.
• Conduct nutritional assessments using RAI MDS 2.0, identifying individualized resident needs and developing targeted interventions.
• Develop, implement, and evaluate nutritional care plans, including identification of nutritional concerns, goals, and interventions for all residents.
• Maintain effective communication with the Dietary Manager regarding resident needs and menu planning.
• Review seasonal menus for nutritional adequacy and provide written approval.
• Educate residents, family members, and team members on nutrition and hydration recommendations.
• Participate in interdisciplinary committees related to resident care, facility operation, and quality indicators.

Qualifications:

• Registered Dietitian, eligible for membership with the College of Dietitians of Ontario.
• Extensive knowledge of geriatric nutrition.
• 3-5 years of experience in a long-term care setting, preferred.
• Proficiency in Microsoft Office and internet applications, with experience in RAI MDS 2.0 and PointClickCare an asset.
• Excellent organizational, time management, and multi-tasking skills.
• Knowledge of Occupational Health & Safety practices, principles, and legislation.
• Excellent oral and written communication skills.
• Customer-focused attitude, with demonstrated ability to build and maintain effective team relationships.
