We are seeking a highly skilled Pharmacist I to join our team at Alberta Health Services. As a key member of our healthcare team, you will play a vital role in providing direct patient care and managing medication therapy for patients in accordance with our clinical expectation standards.
Key Responsibilities- Assess patients for potential or actual drug-related problems and resolve or prevent drug-related problems to optimize patient outcomes.
- Collaborate with other healthcare providers to ensure that medications are used safely and appropriately.
- Document all direct patient care activities in the patient's Health Record and participate in organizational practices, drug stewardship, and quality improvement initiatives.
- Obtain and/or incorporate additional prescribing wherever practice allows and participate in the collection of key performance indicator data and clinical workload measurements.
- Maintain their own professional development and participate in the precepting and mentoring of other staff and students.
- Bachelor of Science in Pharmacy.
- Active or eligible for registration with the Alberta College of Pharmacy (ACP).
- Ability to use computer medication order entry systems and access and proficiently use drug information resources and word processing.
- Fluency in written and verbal English language.
- Ability to incorporate AHS organizational values into person's work, actions, and decisions.
- Recent integrated clinical pharmacist patient care experience, specifically within an addiction and mental health program, or Large Acute Care hospital experience delivering proactive, integrated clinical practice.
- Additional Prescribing Authorization/PRAC ID and ability to administer injections.
- IV additive knowledge/TPN knowledge/Chemotherapy certification.
