**Job Description:
Key Responsibilities- Provide assessment and treatment of patients and clients of all ages with cognitive and/or physical impairments in the client's home and/or the clinic setting as per therapy protocols and best practice guidelines.
- Develop and implement individualized treatment plans to meet the unique needs of each patient or client.
- Collaborate with other healthcare professionals to ensure comprehensive care and support for patients and clients.
- Communicate effectively with patients, clients, and healthcare teams to ensure seamless care and support.
- Stay up-to-date with the latest research, best practices, and industry developments to provide evidence-based care.
- Bachelor of Science in Physical Therapy
- Valid Class 5 driver's license
- Registered/eligible with the Saskatchewan College of Physical Therapists (SCPT)
- Strong interpersonal skills
- Positive approach to deal with challenging situations with empathy and sincerity
- Ability to prioritize work
- Ability to work independently and as a member of a multi-disciplinary team
