- Minimum of 1 year experience in a Care Coordination role, preferably with government contracts.
- Exceptional customer service skills, ensuring a positive experience for clients and their families.
- Able to manage multiple tasks efficiently in a dynamic setting.

- Willingness to work diligently to achieve excellence and implement innovative solutions.
Primary Responsibilities
- Oversee community care coordination and file management.
- Facilitate care for families and loved ones by collaborating with our skilled Community Care Professionals.
- Maintain ongoing communication with field staff and clients to ensure high-quality service delivery.
- Utilize advanced scheduling technology to optimize care assignments.
- Engage in daily meetings with the Home Care Manager to prioritize tasks and provide updates on progress.
- Participate in weekly discussions with the Division Manager to align on priorities and track progress.
- Assess client service needs in collaboration with the Home Support Supervisor, ensuring appropriate staff assignments based on client requirements.
- Monitor and assist with referral processes and ongoing care assignments as required.
- Collaborate with case managers and government programs as necessary.
- Input client schedules into the system and maintain accurate scheduling records.
- Manage phone communications and task creation for field staff scheduling.
- Support the recruitment and orientation of field employees as needed.
- Document all communications from field staff promptly and accurately.
- Assist in training and mentoring new team members as required.
- Conduct follow-ups with clients after initial service to ensure compatibility with care staff.
- Address and document any client or staff concerns promptly, maintaining confidentiality.
- Ensure timely and accurate monitoring of technology platforms used by care professionals.
- Provide courteous responses to all client inquiries and prepare necessary documentation.
Training and Development
- Develop and update procedural documentation related to job responsibilities as needed.
- Adhere to the company's Mission and Code of Conduct in all activities.
- Perform additional duties as required.
- Recognize personal strengths and areas for improvement, seeking support for development.
