Registered Nurse/Case Manager

The Registered Nurse/Case Manager will be a key member of the Interior Health Authority's multi-disciplinary care team, providing assessment, treatment, and case management services to individuals with serious mental health problems, substance use issues, and developmental disabilities.
Key Responsibilities:- Provide assessment, treatment, and case management services to clients with complex needs.
- Collaborate with the care team to develop and implement individualized treatment plans.
- Provide education and support to clients and their families to promote recovery and independence.
- Work with community resources and services to ensure clients receive timely and responsive support.
- Participate in team-based care coordination to ensure comprehensive and coordinated care.
This is a relief part-time position (0.50 FTE) that requires the ability to work independently and as part of a team. The successful candidate will have excellent communication and interpersonal skills, as well as the ability to work in a fast-paced environment.
What We Offer:- A competitive hourly wage of $41.42 - $55.91 per hour.
