Nurse Practitioner

We are seeking a compassionate and skilled Registered Practical Nurse to join our team at SE Health. As a Registered Practical Nurse, you will have the opportunity to make a meaningful difference in the lives of our clients and their families.
About the RoleAs a Registered Practical Nurse, you will be responsible for providing high-quality patient care in a community setting. You will work collaboratively with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of our clients.
Responsibilities- Conduct comprehensive client assessments to identify their physical, emotional, and social needs
- Develop and implement individualized care plans that address the client's unique needs and goals
- Provide direct patient care, including administering medications, wound care, and other treatments as prescribed
- Collaborate with the healthcare team to ensure seamless care transitions and coordination
- Support clients and their families in achieving their health and wellness goals

To be successful as a Registered Practical Nurse at SE Health, you will need to have:
- A current registration with the College of Nurses of Ontario
- Canadian Nurses Association (CNA) specialty certification an asset
- Enthusiasm and love of community care and passion for evidence-informed practice & excellence in care
- A current CPR certificate
- A valid driver's license and vehicle (travel required)
- Updated immunization records, clear criminal background check, and Vulnerable sector check are required upon hire
