  • Completing comprehensive client assessments and developing personalized care plans
  • Providing direct care to clients in their homes and communities
  • Collaborating with other healthcare professionals to ensure seamless care transitions
  • Supporting clients through care transitions by connecting them with community resources and services

To be successful in this role, you will need:

  • A current registration with the College of Nurses of Ontario
  • Canadian Nurses Association (CNA) specialty certification an asset

  • A current CPR certificate
  • A valid driver's license and vehicle (travel required)
