- Minimum of 1 year experience in a Care Coordination role, preferably with government contracts.
- Strong customer service skills with a focus on delivering a positive experience.
- Ability to manage multiple tasks effectively in a dynamic environment.
- Positive interpersonal skills to engage with clients, families, and care teams.
- Commitment to excellence and innovation in care delivery.
Primary Responsibilities
- Oversee community care coordination and maintain accurate file management.
- Facilitate care arrangements for families and clients by collaborating with our skilled Community Care Professionals.
- Maintain open communication with field staff and clients to ensure satisfaction and quality of service.
- Utilize advanced scheduling technology to optimize care delivery.
- Engage in daily meetings with the Home Care Manager to prioritize tasks and monitor progress.
- Conduct weekly reviews with the Division Manager to align on priorities and ensure timely service delivery.
- Assess client needs in collaboration with the Home Support Supervisor and schedule appropriate staff accordingly.
- Ensure consistency in caregiver assignments and service coordination.
- Monitor and manage new referrals and ongoing care assignments as per government reporting requirements.
- Collaborate with case managers and funding programs as necessary.
- Maintain accurate scheduling information and perform data entry tasks.
- Handle scheduling communications and create tasks for field staff efficiently.
- Assist in the recruitment, orientation, and training of field employees as needed.
- Document and follow up on client and staff interactions to ensure compatibility and satisfaction.
- Maintain confidentiality of all client and corporate information.
- Monitor technology platforms to ensure timely attendance and proper application usage by Care Professionals.
- Respond promptly to customer inquiries and prepare necessary documentation.
Training and Development
- Develop and update procedures related to the responsibilities of this position.
- Adhere to the company's Mission and Code of Conduct in all aspects of work.
- Perform additional duties as required.
- Recognize personal strengths and areas for improvement, seeking assistance as needed.
