Job Summary

Halton Healthcare is seeking a highly skilled and experienced Geriatric Nurse Practitioner to join our team. As a key member of our interprofessional team, you will work closely with physicians and other healthcare professionals to provide high-quality, patient-centered care to our geriatric patients.

Key Responsibilities
  • Provide comprehensive geriatric care to patients in both inpatient and outpatient settings, including health assessment, diagnosis, and treatment planning.
  • Collaborate with geriatricians and other healthcare professionals to develop and implement individualized care plans.
  • Conduct comprehensive initial patient assessments and establish individualized treatment and care plans for complex geriatric patients.
  • Provide direct, specialized geriatric care and consultation to patients, families, healthcare professionals, and community partners in complex situations.
  • Support the development and implementation of quality management programs and participate in strategic planning for the delivery of geriatric care.
  • Contribute to the education and professional development of nursing and allied health staff, students, and the professional community.
  • Participate in academic and research projects to advance the field of gerontology.
Requirements
  • Current registration in the Extended Class with the College of Nurses of Ontario.
  • Master's Degree in Nursing.
  • Minimum 3 years of nursing experience, with geriatric expertise preferred.
  • CNA Specialty Certification in Gerontology preferred.
  • Eligible for cross-appointment to an academic institution preferred.
  • Membership in related professional associations required.
  • Innovative, creative, and professional role model.
  • Ability to prioritize and organize multiple demands.
  • Ability to work autonomously.
  • Proficient with Microsoft Office applications.
  • Able to meet the physical demands of the position.
  • Satisfactory employment and attendance record required.
