Supportive Housing Program Coordinator

The Support Service Coordinator is a key member of the supportive housing team, responsible for coordinating services for seniors at a supportive housing site in Georgetown. This role involves assessing client eligibility and needs, developing care plans, coordinating service delivery, supervising contracted staff, and crisis intervention.
Key Responsibilities
- Assess client eligibility and needs, and develop individualized care plans
- Coordinate service delivery, including personal support and homemaking services
- Supervise contracted staff, ensuring high-quality service delivery
- Provide crisis intervention and support as needed
- Collaborate with community partners to ensure seamless service delivery
Requirements
- College-level diploma in the human services field
- Education and/or experience in gerontology
- Knowledge of systems and services for seniors
- Experience working in a community setting
- Excellent communication and interpersonal skills
- Strong organizational and analytical skills
