Job Summary

We are seeking a highly skilled and experienced Licensed Practical Nurse (LPN) to join our Home Support Team as a Team Leader/Primary Care Nurse. In this role, you will be responsible for coordinating Home Support Services, supervising Community Health Workers, and providing training and evaluation of personal care techniques.

Key Responsibilities
  • Coordinate Home Support Services, including supervising Community Health Workers and overseeing the implementation of client care plans.
  • Provide training and evaluation of personal care techniques to ensure high-quality care for clients.
  • Function as a member of the interprofessional team, applying best practice to provide competent, safe, and ethical care for people and their families in various settings.
  • Perform assessments, plans, and implements personal and nursing care to clients, including administering oral, subcutaneous, and intramuscular medications in accordance with pharmaceutical and organizational policies and procedures.
Requirements
  • Registration with BC College of Nurses and Midwives as a practicing LPN registrant.
  • Two (2) years of recent, related experience, including one (1) year in a supervisory role, or an equivalent combination of training and experience.
  • Current valid BC Driver's License.
