Job Title: Social Worker - Mental Health Specialist

North York General Hospital is seeking a highly skilled Social Worker to join our Mental Health Inpatient team. As a Social Worker - Mental Health Specialist, you will provide patient-centered care to individuals and families in our Mental Health Inpatient units across the lifespan.

Key Responsibilities:
  • Conduct comprehensive biopsychosocial assessments to identify patients' emotional, physical, cognitive, and social needs, risks, and strengths.
  • Develop and implement individualized care plans in collaboration with the multidisciplinary team and community partners.
  • Provide guidance and support to patients and families in developing crisis plans and coping skills to promote wellness and recovery.
  • Collaborate effectively with the healthcare team to ensure seamless care transitions and discharge planning.
  • Maintain excellent knowledge of hospital and community resources, and provide advocacy and support to patients and families as needed.
Requirements:
  • Master's Degree in Social Work from an accredited program.
  • Registration with the Ontario College of Social Workers & Social Service Workers (OCSWSSW).
  • Minimum of one year of experience as a staff Social Worker in an acute care setting.
  • Geriatric experience is an asset.
  • Excellent clinical skills, with ability to work effectively in a fast-paced environment.
  • Proficient in computer skills, including MS Word, internet searches, and patient information systems.
