We are seeking a highly skilled and compassionate Patient Access Navigator to join our team at Nova Scotia Health. As a key member of our Mental Health and Addictions team, you will play a vital role in facilitating access to mental health and addiction services across the province.
Your primary responsibility will be to provide telephone-based support and guidance to individuals seeking mental health and addiction services. You will utilize your expertise in motivational interviewing, engagement techniques, and harm reduction strategies to assess client needs and provide appropriate referrals to community resources and mental health teams.
As a Patient Access Navigator, you will work closely with clients to identify their strengths and challenges, and develop personalized plans to support their mental health and well-being. You will also collaborate with community partners and mental health teams to ensure seamless transitions and continuity of care.
We are looking for a candidate with a strong background in social work, preferably with experience working with individuals with mental health and addiction concerns. You will possess excellent communication and interpersonal skills, with the ability to work effectively with diverse populations and stakeholders.

About You- Master's degree in Social Work or related field
- Registration with Nova Scotia College of Social Workers required
- Minimum 3 years of experience in mental health and addiction services
- Strong knowledge of motivational interviewing, engagement techniques, and harm reduction strategies
- Excellent communication and interpersonal skills
- Ability to work effectively with diverse populations and stakeholders
- Long assignment full-time position; 75 hours biweekly
- Approximately 12 months
$42.89 hourly
Successful candidates may be eligible for our benefits package, which includes health, dental, travel, long-term disability, and life insurance coverage, as well as a defined benefit pension plan.
