We are seeking a skilled Clinical Social Worker to join our team at Interior Health Authority. As a key member of our healthcare team, you will provide comprehensive assessment, care planning, and treatment coordination for designated clients and their families.
Key Responsibilities- Provide direct psychosocial care, counseling, system navigation, and advocacy to clients and their families.
- Collaborate with other healthcare providers and the multidisciplinary team to develop and implement care plans.
- Facilitate client self-management and provide education on relevant specialized complex care resources.
- Develop and maintain social work services to maintain or improve the health and social well-being of the identified population.
- Master's degree in Social Work or a related field.
- Experience in clinical social work, preferably in a healthcare setting.
- Strong assessment and treatment planning skills.
- Excellent communication and interpersonal skills.
- Ability to work collaboratively as part of a multidisciplinary team.
